Niagara’s Let’s Go Home program has received an additional $125,000 in one-time funding from the Ministry of Health, extending support through March 31.
The funding builds on the program’s existing $500,000 base and will help serve at least 50 more clients as demand continues to rise.
The program supports seniors 65 and older, including those living with dementia, by helping them safely return home after a hospital stay and avoid unnecessary admissions through coordinated community services.
